Do you Accept Insurance?

Yes! I’m in-network with Unitedhealthcare, Oxford, Aetna and Northwell Direct.

What if I have insurance that you don’t accept?

I will happily provide you with a superbill and my W9 to submit to insurance companies so you can use your out-of-network benefits, if applicable.

What are your rates?

Initial Assessment (CPT code 90791)- $225

45 minute sessions (CPT code 90834) (38-52 minutes)- $175

60 minute session/EMDR Session (CPT code 90837) (53+ minutes)- $200

What is your cancellation policy?

If you need to cancel or reschedule our sessions, I ask for 24 hours notice.

I do realize that sometimes life happens and we can’t always predict what may arise. I do my best to work with everyone on a case-by-case basis about this policy but will charge a $100 cancellation fee in certain cases.

For no-shows, I will charge either my full contracted rate with your insurance company or my full fee for private pay clients. **These charges are not reimbursable by insurance companies.

Good Faith Estimate

If you do not have insurance, or are choosing not to use your insurance, beginning January 1st, 2022 you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.  This "Good Faith Estimate" will be provided verbally and in writing upon scheduling services or upon request.

Common Insurance Terms

In-network (INN) I am contracted with this insurance company and accept these plans.  You will only be responsible for paying your copay, or my contracted rate if you have a deductible to meet first. I submit claims directly to your insurance company.

Out of network (OON) I am not contracted with these insurance companies.  Depending on your specific plan, you may have out-of-network benefits available to you. If so, you will have to meet your OON deductible first before the plan starts to pay its share.  You’ll pay my full fee directly, and I’ll provide you with a superbill and my W9 to send to your insurance company.  I am able to submit claims directly in certain situations. 

Superbill  An itemized list detailing the sessions we’ve had. Included on this bill are the following; Dates of insurance, CPT code which shows the length of treatment and type of treatment provided (i.e., individual psychotherapy), mental health diagnosis, rate charged, rate paid, along with my professional information such as name, NPI # and tax ID #. 

Deductible A specific dollar amount that you need to pay out-of-pocket before your insurance company picks up its share of costs.  

Co-insurance Once you meet your deductible, this is the amount you’re responsible for until you meet your out-of-pocket max.

Out-of-pocket (OOP) Max  The most that you will be required to pay out-of-pocket until your insurance picks up 100% of the costs.  Copay’s, in-network deductibles, and co-insurance all count towards your OOP max for IN-NETWORK services.
